Oh, I hate that too. They stick an insult into the mix in such a way that you're never quite certain if they meant it or if it was just a poor choice of words.

I have a "friend" who does that all of the time with a very innocent look on her face. It's like she is daring me to call her on the insult so she can be surprised that I "took it the wrong way." Grrrrrrrrr!
